The POLST form guides discussions between patients, their families, their physician, and their healthcare team about treatment wishes in instances of serious illness. POLST transforms those wishes into physician orders, which are actionable and to be respected across the continuum of healthcare settings. AKPOLST is launching akpolst.org, a new website housing the POLST form and resources for clinicians, patients and families, and long-term care facilities.

The webinar will be led by Dr. Ursula McVeigh. Dr. McVeigh joined Providence Alaska in 2015. She is the Executive Medical Director of Hospice and Palliative Care in Providence Alaska Medical Group and Program Director of the Providence Alaska Hospice and Palliative Medicine fellowship. She has been serving on the DHSS POLST Taskforce since 2016 and is the lead of the AK POLST Education Workgroup. Prior to moving to Alaska, Dr. McVeigh was a palliative care clinician educator and Medical Director at University of Vermont Medical Center.
