AHHA ASSOCIATE MEMBERSHIP APPLICATION

AHHA membership includes Associate Member organizations - physician groups, clinics, health care consultants, insurance companies, community partners, and other businesses that support the health care industry - whose purpose and objectives are aligned with AHHA's vision and mission.  To be considered for associate membership, please complete the application below - membership is confirmed through election by AHHA's Executive Committee.

We have three levels of membership:

Community Partner (must be non-profit organization or government agency) - $750/year
Gold Corporate - $1850/year
Platinum Corporate - $3500/year

Memberships are prorated, based on the date of approval, and membership renewals are issued in January for each subsequent year.

AHHA does not endorse or recommend associate member organizations, nor do we enter into exclusive partnerships with any vendors or service providers who hold associate membership.
